References (2 references required): You can find the link for the reference form on the FHIMA website. Go to the "Students" section on the FHIMA homepage. Click on "Scholarships" to find this link. Please copy and paste the Reference Form link into an email and send the email with an explanation/instructions to the person you wish to serve as a reference for you. Provide the person with the deadline to have it done by and you should follow up with them to make sure they have completed the reference.

Educational Enrollment Verification form: You can find the link for the enrollment verification form on the FHIMA website. Go to the "Students" section on the FHIMA homepage. Click on "Scholarships" to find this link. Please copy and paste the enrollment verification form link into an email and send the email with an explanation/instructions to your HIM Program Director. Provide the person with the deadline to have it done by and you should follow up with them to make sure they have completed the verification form.

Please note: Incomplete applications (including missing information on the application, missing required documents such as reference forms, enrollment verification) will disqualify your application! Make sure you follow up with all references and your Program Director (for enrollment verification) and make sure they understand FHIMA needs this information BEFORE the posted deadline! FHIMA will not follow up on any incomplete applications!
